6K a year We are recruiting for FULL TIME Case Managers to work in either our Adult Partial Care or Outpatient programs Partial Care program hours:
Monday-Friday 8:
45am-4:
30pm Outpatient Program hours - varies and can include some evenings up to 8pm and Saturday til 4pm.
Should have experience/knowledge networking with community health agencies.
Should understand and be comfortable working with public speaking, Medicare/Medicaid.
About us Established in 1958 as the North Essex Child Guidance Clinic, Northwest Essex Community Healthcare Network, Inc.
(NECHN) began serving emotionally disturbed children and their families from the towns of Belleville, Bloomfield and Nutley.
Due to growth, in 1964 the staff could no longer be contained in its original site on the premises of Clara Maass Hospital, and as a result, the Essex County Board of Chosen Freeholders provided space for our Agency on the grounds of the then Essex County Geriatric Center.
At this time, Essex County residents began receiving psycho therapeutic services for mental health issues at this Agency.
Northwest Essex offers a comprehensive range of services which includes Outpatient, Co-Occurring, Addiction, Bilingual and Partial Care.
Treatment is individualized according to each person's needs and utilizes a recovery and wellness approach to maximize an individual's potential.
' ' POSITION
Summary:
This position provides Case Management Services to consumers to help consumers gain access to resources and services needed to meet their mental health needs, achieve recovery, and live independently in the community.
Services are coordinated with internal and external providers to help each consumer achieve personalized case management needs.
1.
Provide assistance and referrals and care coordination in the areas of housing, applying for welfare, Medicaid, supportive education, supportive employment and other needed services.
2.
Maintains a caseload of case management consumers.
3.
Responsible for documenting case management progress notes in a timely fashion.
4.
Completes initial applications (phone screens) of referrals on a scheduled basis.
5.
Completes documentation of all initial applications within 8 hours.
6.
Completes Consumer Advisory Form and reviews areas on the form with the consumer before the consumer signs the form.
7.
Provide outreach for consumers that have missed their appointments.
8.
Attend multi-disciplinary team meetings to provide input and obtain feedback about consumer cases.
9.
Develop community contacts to secure community referrals to ensure consumers needs are met.
10.
Utilize person centered approach to provide assistance to consumers to meet their prioritized needs.
11.
Attend trainings as appropriate to the position or for requirements for the agency to maintain positive open and respectful communications with consumers, families and other involved persons.
MINIMUM REQUIREMENTS BA in human service field at least one year experience in crisis counseling Organized and demonstrates strong leadering and interpersonal skills Ability to work with computers and necessary software Competitive Salary This is a Union Position that includes salary increases as per union contract
